Fall prevention from the nurse's point of view

The bachelor thesis focuses on fall prevention from the perspective of the general nurse. General nurses encounter falls very frequently with health care providers and should be able to act on this issue in both the prevention and management of a fall. This thesis consists of a theoretical and a research part. The theoretical part deals with the characteristics of falls and risk groups of patients and other internal and external risk factors that occupy an important role in this issue. It also discusses the role of the general nurse in fall prevention as an integral part of the practice of the profession. The thesis also deals with selected aspects of fall prevention and follow-up of fall occurrence and consequences in patients. It was also important to mention the scales used to assess falls or to reduce the risk of falls. For the research part of this bachelor's thesis, a qualitative data collection method was chosen by using the technique of semi-structured interviews conducted with general nurses of selected wards of internal type of health care providers. The aim of the research part was to find out how general nurses identify patient groups at risk for falls. Also, whether general nurses implement preventive measures for fall occurrence and how they proceed when a fall occurs. Last but not least, it was important to find out how general nurses proceed when a patient is discharged to home care.
Falls in Geriatric Patients at the Follow-up Care Department

This diploma thesis is dedicated to falls in geriatric patients in the aftercare department. Thesis Objectives: To carry out a complex analysis of multifactorial causes in chosen cases of geriatric patients that occured during hospitalization in a chosen aftercare workplace from 2020 through 2021 in Nemocnice České Budějovice, a.s. To carry out SWOT analysis of a chosen aftercare workplace in Nemocnice České Budějovice, a.s. with the objective to evaluate interventions used in preventing falls of patients hospitalized in the given workplace. Suggestion of recommendation to improve current practice of the chosen aftercare workplace in Nemocnice České Budějovice, a.s. in preventing falls in hospitalized patients. Methodology and Research File: To reach the objectives, three research questions were chosen. What are multifactorial causes of chosen cases of falls in geriatric patients in the chosen aftercare workplace? What interventions are used to prevent falls in geriatric patients in the chosen aftercare workplace? What interventions might improve current practice in preventing falls in hospitalized geriatric patients in the chosen aftercare workplace? To implement a survey, a qualitative research strategy was chosen. For case study purposes, the following data collection techniques were used - medical documentation content analysis, reporting adverse events, controlled documentation etc., semi-structured interviews with doctors in specific fall cases. In the following part of the survey, where we were finding out about the intervention used and intervention meant for improving the current practice in fall prevention, semi-structured interviews were carried out with doctors and nurses. Interviews with the nurses were completed by participant observations. Based on the survey results, a SWOT analysis was processed. The research file consisted of 14 chosen falls that happened in hospitalized geriatric patients in the aftercare department from 2020 through 2021, of 12 nurses working in the aftercare department, of 9 doctors that were informed of a fall. A deliberate choice of both nurses and doctros was used, the criterium for being included in the research file was the minimum of a year-long practice in the aftercare deparment. The obtained data was processed using the pen-and-paper method, open coding, and data was subsequently categorized and noted down into tables and schemes. Results: Complex analysis of multifactorial causes of chosen fall cases proved that falls happened due to internal as well as external factors. The risk of a fall was increased by the pateints´ age, morbidity, polypharmacotherapy, cognitive deficit, sarcopenia, fragility syndrome, instability, bed siderails lifted, and human resource failure. Based on the results, we carried out SWOT analysis and we suggested a recommendation to improve the current practice in the chosen aftercare workplace. Practice Recommendations: Based on the survey, we would recommend for a repetitive evaluation of the risk of patients falling to be carrried out. Another recommendation would be educating the nursing staff in the area of fall prevention, in the area of communicating with geriatric patients and patients suffering from dementia. Further on, we would recommend ensuring the use of suitable positioning aids and securing a movement-detection system inside the bed. Last but not least, we would recommend increasing numbers of nursing staff.
Evaluation of the effects of different walking types on the postural stability in healthy seniors - a systematic review.

Title: Evaluation of the effects of different walking types on the postural stability in healthy seniors - a systematic review. Objective: The main objective of this diploma thesis is the evaluation of the impact of different types of walking on the postural stability in healthy elderly, based on the available resources corresponding to the established inclusion criteria. Force platforms were used as the tool to evaluate and to objectify postural stability in all studies included. These platforms are capable of measurement the level of postural stability by recording stance excursions in multiple directions. This thesis makes efforts to review the level of the possible future inclusion of different walking types in the process of physical therapy among older patients, regarding to improve the postural stability as well as the fall prevention in these patients. The next goals of this diploma thesis are to determine which type of walking appears to be the most efficient comprising the fall prevention as well as to assess the optimal frequency of the ordinary walking performance to achieve the same objective. Furthermore, the work is focused on summarizing current knowledge of this issue. Methods The thesis is processed in the form of literature review and has an analytical-descriptive character. The studies used meet the predetermined criteria. The thesis is divided into several parts. In the first part, which is called theoretical basis, are described mainly physiotherapeutic methods and techniques, which are currently used to ensure the prevention of falls in seniors. The falls of seniors at home environment

This bachelor thesis focuses on one of frequently occuring problems affecting seniors which influence the senior´s and their family´s lives very negatively. It is falls of seniors in the home environment, usually leading to a long-term hospitalization of the seniors, often leading to their health-condition deterioration, or, in the worst cases, to the seniors´ death. That is why it is important for seniors and their families to be sufficiently informed about ways to prevent falling, and what measures to adopt to avoid falling. This thesis consists of two parts. The introducing chapter of the theoretical part describes the scientific fields of gerontology and geriatrics, and defines the terms of aging and old age. Further, the ways of geriatric patients examination are described. Falls, their etiology, phenomenology, their results and prevention are characterized in the following chapters of the theoretical part. The research part of the thesis defines four aims. It was trying to ascertain what specific circumstances to falls of seniors in the home environment there are, what consequences there are from the seniors´ and their family members´ point of view, whether seniors having suffered from a fall in the past have taken any preventive measures to avoid more falls, and what opinion is held by their family to fall prevention. 5 seniors having suffered a fall in the home environment, and 5 of their family members took part in the research. When processing the data obtained via interviewing the respondents, 5 categories and 6 sub-categories were created. The results have proven that the fall usually influences the future life of the given senior negatively. All of the questioned respondents had stated certain fear of fall repetition in the interview, some preventive measures were taken by some of the respondents only. Based on this bachelor thesis, a short educative material has been created, which may be further used as an educative leaflet serving to pass on practical information on preventing falls of seniors.
Fall prevention and affecting the quality of gait among the elderly by increasing the afferent signals from the foot

This thesis examines the influence of afferent information coming from the feet to the quality of walking. Bipedal locomotion is a movement that follows people since the first year of life. In childhood, it is a sign of our progressive growth and maturation of the nervous system and in old age it is similar. Often it is the walk that determines the ability of seniors to take care of themselves. With age, it is physiological that the walk changes. Whether it is depending on the diseases, which in elderly age often merge into chronicity, or for example pathological patterns of movement which the body can not compensate so well anymore. Impaired mobility, which goes hand in hand with walking, brings the risk of falling. Fall is one of the most serious injuries for seniors and its consequences can seriously jeopardize self-sufficiency and mobility of seniors. The risk of falling appears just for older people who are no longer able to adapt quickly to changes. The foot is a significant part of the stabilization system of the body. It mediates the first contact with the pad and informs us about its properties. This information is then transmitted to the upper decks of the central nervous system, which then better adapts movement to the pad surface. The amount of afferent information depends on many factors. The most important is the shoe that absorbs a large amount of afferent information from the foot and feet can not fully perform their function of gathering the information. In the theoretical part of my thesis, I discussed changes in physiological, social and psychological, the person goes through the aging process. Then I dealt with falls and their causes, consequences and prevention. In the third chapter I worked sole topic, its construction and the most frequent pathology. And in the last part I focused on walking, its development stages, types, and how should ideally look like. The aim of my thesis is to evaluate how much influence will stimulating the soles of the feet have for walking. Stimulation soles lasted for three months in the form of soft technology and mobilization leg joints, hardening feet hot and cold water, skin stimulation (sifting through the legs in a container with legumes), practicing active (small) foot and eventually coaching momentum and coordination feet by painting or writing with feet (pencil between 1st and 2nd finger). Another aim of my thesis is to identify preventive measures against falling for individual seniors. For this it is necessary to focus on the weaknesses of the individual and to assess its risk of falling. Long-term therapy should be designed so that it is manageable for seniors with regard to his health and ability. In the practical part was used qualitative research. As data acquisition techniques were used anamnesis, kinesiology analysis, examination of walking, fine motor skills and foot examinations of perception. Research was attended by four seniors, three women and one man in age ranges 70-80 years. Therapy carried every week for three months, in the end each proband completed twelve therapies. Each therapy lasted about 30-45 minutes. The results show that it has improved balance especially when standing. Patients gain more stability, two at Romberg III lost titubation and their endurance during solo standing increased. The walk improved, especially in the coordination. In unison, all patients improved fine motor skills of their feet. At the initial examination, only one patient managed to pick up a pen with both feet. At the output, all patients managet it. This work can be used by physiotherapists and other health professionals in nursing homes and similar facilities. It can also be used to increase brand awareness of senior's family members or as injury prevention in the home environment.
Accident prevention in patients with rheumatoid arthritis.

The topic of the bachelor thesis is fall prevention in patients suffering from rheumatoid arthritis. The topic would like to highlight fall prevention that is very important in patients with this disease. Rheumatoid arthritis is a chronic inflammatory disease of connective tissues of the whole organism. The main manifestations of the disease are affected joints, muscles and ligaments. This leads to significantly reduced mobility of joints. However, it can gradually affect other organs of human body, such as heart, respiratory system etc. The quality of life of these patients depends on at what stage of the disease the patient is. The disease gradually limits the clients´ ability of self-care and increases their dependence on close family and friends. Morning stiffness often appears and swelling around the affected joints increase the risk of falling. The purpose of the treatment is to suppress pains, eliminate stiffness, increase muscle strength and improve function of the muscle. Fall prevention in clients with rheumatoid arthritis is an integral part of the treatment. A fall is an unpleasant experience for a client. It might belong to incidents that worsen and prolong the treatment. Thus, a fall might belong to situations that should be prevented. Due to everyday morning stiffness the clients are under permanent threat of a fall and that is why the treatment and prevention of a fall are very essential. Three objectives were chosen for the practical part of the thesis. The first objective was to find out what measures to prevent a fall the clients with rheumatoid arthritis use. The second objective was to map what preventive procedures are used by nurses in medical facilities to prevent a fall of clients with rheumatoid arthritis. The third objective was to find out what measures preventing a fall would be appreciated by the clients with rheumatoid arthritis. A qualitative method of research was applied. 5 clients with rheumatoid arthritis were one examined group. These respondents were chosen on the basis of pre-set criteria- these were: rheumatoid arthritis, stage of the disease III. and IV. and past history of hospitalization. 5 nurses taking care of clients with rheumatoid arthritis in a medical facility were the other examined group. The method of questioning, the technique of a non-standardized interview was applied. The thesis revealed what kind of aids would be clients appreciated to prevent falling. On the basis of the collected data a draft was designed how to prevent falling in a medical facility for clients with rheumatoid arthritis and for nurses who take care of them. Then a set of ten preventive measures was created for clients with rheumatoid arthritis to prevent falling. These aids might be used in hospitals as a recommendation for nurses how to behave to prevent the clients with rheumatoid arthritis from falling and the nurses should provide clients with complex information how to prevent falling.
